Case Management Services
CSO’s case management program provides one-on-one support to help individuals set and reach their personal and financial goals. Case managers work alongside participants to build financial stability, strengthen problem-solving skills, and connect them with helpful community resources and support.
When funding opportunities are available, our role goes beyond providing financial assistance. Through our case management services, we work with participants to use those resources in ways that promote long‑term stability rather than dependence, supporting sustainable progress and lasting self‑sufficiency.

Building Stability Through Case Management

Since 2022, Deserea has been working with Amy in our program, showing incredible determination and resilience. We’re proud to celebrate her successful completion of the CSBG Self-Sufficiency Program—and the achievement of every personal goal she set for herself.
Deserea stated, "Case management has helped me become more self-sufficient and confident in my abilities. I’ve learned how to handle many things on my own, and also know when to reach out for help. Through this process, I have gained the skills to prepare for and succeed in job interviews, as well as how to create a professional resume. These tools have given me a stronger foundation for reaching my goals."

After relocating to Shamokin from Allentown, Johanna and Marcos came to Central Susquehanna Opportunities (CSO) seeking help with housing and employment. At the time, they were staying with friends in an unstable living situation and needed support to get back on their feet.
Through CSO’s case management services, the couple worked with staff to develop a plan focused on employment and housing stability. Johanna soon secured a job, and shortly after, CSO helped them locate a rental home—an important milestone that provided the stability they needed to move forward.
Not long after the move, Marcos also found full-time employment close to their new home. With both working, their situation quickly improved. Community partners, including DIG Furniture Bank and AGAPE, helped provide furnishings so they could turn their house into a comfortable home.
With continued support from their case manager, Amy, Johanna and Marcos worked on budgeting and maintaining stability. Through determination and community support, they were able to build a stable life and create a hopeful new beginning in Shamokin.
